    An exclude * wildcard always overrides any include filters. To include just the one file in the root folder, drop the exclude filter and specify the file-specific include filter:

    Include files: .\index-dist.htm

    Such a filter implicitly excludes all other files.

    Exclude the base folder's files by adding ".\*" as an "Exclude Files" filter.

    How do I include JUST the specified folders and their contents? Despite that I put into Include Folders a list that excludes the base folder, FC includes the base folder files.
